Applicant certifies you are an authorized representative with authority to enter into this agreement and the information contained in this Application and any attachment is true, correct and complete. Payment terms are net 30 days. By signing this application you consent to Golden Ratio Logistics LLC to contact any or all credit and bank references provided. Applicant affirms that there are no open judgements, suits, or liens against your company. Applicant understands that Golden Ratio Logistics may report payment experiences to credit reporting agencies. Applicant understands that motor carriers are required to maintain 100k of cargo insurance and if value of the load tendered exceeds 100k you must notify us before booking the load so we can secure carrier with correct amount of coverage. If credit is extended, you agree to be bound by all of the terms and conditions on Vendor’s invoices.
